If you wish to refer a young person to our service, please print
and complete the referral form below and fax it to us on (02) 6338
1199. We will promptly attend to this referral and notify you
following intake.
headspace referral form: for GP's and
all other service providers including teachers, counsellors
and community groups.
Self referral: If you are a young person between the ages of
12-25 and wish to refer yourself please call (02) 6338 1100
(Bathurst), (02) 6342 6186 (Cowra), or email headspace@hscw.org.au, or
visit one of our sites to make an appointment.
Family/Friend referral: If you wish to refer a family
member or friend between the ages of 12-25, please call (02)
6338 1100 (Bathurst), (02) 6342 6186 (Cowra), or email headspace@hscw.org.au, or
