Make a referral

If you wish to refer a young person to our service, please print and complete one of the referral forms below and fax to us on (02) 6923 3145. We will promptly attend to this referral and notify you following intake.

Alternatively, you can contact our office during office hours (9am to 5pm Monday to Friday) to talk to one of our case managers about the referral process. Please keep in mind that the person you are referring must know and consent to the referral.

Self referral: If you are a young person between the ages of 12-25 and wish to refer yourself, please send an email to myheadspace@riverinaheadspace.com.au. Or you can call us directly on (02) 6923 3170.

We will contact you during our normal business hours to arrange an appointment.
