Referral forms
We now have two referral forms for headspace counselling services available for download.
One is for Doctors to use (GP Referral Form) and the other one is for
everybody else (Non GP Referral Form). So pick the form that's
for you, download it, print it off, fill it in and either fax it,
post it or drop it into headspace. Please
do not email your referral to us.
